About Success at Home

Success at Home provides RN-led support for individuals and families navigating care needs at home. We help make sense of medical information, medications, home safety concerns, and next steps — offering clear guidance that fits real life, not just systems.

Our work is grounded in clinical expertise, practical problem-solving, and respect for each client’s goals, preferences, and circumstances.

Our Approach

We focus on guidance, coordination, and continuity — helping clients and families feel informed and supported at home.

RN-led clinical perspective with real-world practicality

Personalized guidance — not one-size-fits-all checklists

Care coordination across providers, medications, and equipment

Support for families near or far

Clear recommendations that you can actually act on

We’re here to reduce overwhelm and help you move forward with a plan.

Our Leadership

Success at Home is led by professionals with complementary clinical and operational expertise.

Ralene Vaughn, BSN, RN, AMB-BC – Clinical Director

As Clinical Director, Ralene oversees nursing assessments, care planning, and clinical oversight to ensure services are thoughtful, appropriate, and grounded in sound nursing judgment. Her work emphasizes safety, clarity, and individualized support for clients and families navigating care at home.

Ralene has over 25 years of nursing experience and is a Registered Nurse with a Bachelor of Science in Nursing (BSN) and board certification in Ambulatory Care Nursing (AMB-BC). She is also a certified Senior Home Safety Specialist, with specialized training in identifying environmental risks, fall prevention strategies, and home modifications that support safe aging in place.

Andrea Caruso – Director of Operations

As Director of Operations, Andrea brings both professional expertise and lived experience to her work. Having navigated complex care needs herself, she understands firsthand how overwhelming medical systems, coordination gaps, and accessibility challenges can be. This perspective informs her commitment to clear communication, thoughtful coordination, and practical support for clients and families.

Our Focus

We work with individuals and families who want proactive, personalized support at home — particularly when care involves multiple providers, medications, or changing needs.

Our services are designed to complement existing medical care and community resources, offering guidance and coordination rather than replacing care already in place.

Professional Standards & Privacy

We take privacy, documentation, and professional boundaries seriously. Success at Home uses secure systems and maintains clear standards to support responsible communication and care coordination.

Service Area

We currently serve clients in Skagit, Snohomish, and King Counties, Washington, with plans to expand.
