Our Services
Initial Comprehensive Home Assessment
A 90-minute in-home registered nurse assessment designed to establish a clear understanding of health, safety, and daily living needs.
This visit includes:
Nursing assessment
Home safety evaluation
Medication review
Medical technology support
Written summary with recommendations
*This assessment is required prior to enrolling in ongoing wellness packages.
Ongoing Wellness Support
Ongoing RN-led support for individuals who benefit from regular monitoring, guidance, and care coordination at home.
Family Consultation
Dedicated consultation time for families seeking clarity, guidance, or support navigating care decisions. Available in person or virtually.
Medical Technology Support
Hands-on assistance with medical devices, monitoring equipment, and home health technology to ensure safe and effective use.

How It Works
Step 1: Start with an Initial Assessment
We begin with a comprehensive in-home RN assessment to understand needs, risks, and priorities.
Step 2: Develop a Personalized Plan
Based on the assessment, we provide clear recommendations and identify appropriate levels of ongoing support.
Step 3: Ongoing Guidance & Coordination
For clients enrolled in wellness packages, we provide regular RN visits, coordination support, and plan adjustments as needs evolve.
Pricing & Packages
All services are provided on a private-pay basis and led by a registered nurse.
INITIAL COMPREHENSIVE HOME ASSESSMENT – $325
One-time visit (90 minutes)
Includes nursing assessment, home safety evaluation, medication review, medical technology support, and written summary.
ONGOING WELLNESS PACKAGES
Platinum Plan – $550 per month
Weekly RN visits and priority support
Gold Plan – $300 per month
Two RN visits per month
Silver Plan – $175 per month (minimum 3-month commitment)
One monthly in-home RN check-in
MEDICAL TECHNOLOGY SUPPORT VISIT $150 – 60 minutes
FAMILY CONSULTATION $125 – 60 minutes (virtual or in-person)
