What We Do
Success at Home provides nurse-led, in-home assessments and ongoing care coordination for individuals and families navigating health needs at home. Our services focus on safety, clarity, and continuity — helping clients stay well supported where they live.

Home safety evaluations are conducted by a Registered Nurse who is also a Senior Home Safety Specialist, with training specific to fall prevention, environmental risk identification, and aging-in-place safety.
What We Don’t Do
- We do not provide hands-on personal care (bathing, dressing, toileting).
- We do not replace home health, hospice, or skilled nursing services ordered by a physician.
- We do not provide emergency or on-call medical care.
- We do not offer task-only or hourly caregiving services.
- At this time, we do not manage insurance billing or act as a medical provider for treatment.

How It Works
Step 1: Start with an Initial Assessment
We begin with a comprehensive in-home RN assessment to understand needs, risks, and priorities.
Step 2: Develop a Personalized Plan
Based on the assessment, we provide clear recommendations and identify appropriate levels of ongoing support.
Step 3: Ongoing Guidance & Coordination
For clients enrolled in wellness packages, we provide regular RN visits, coordination support, and plan adjustments as needs evolve.

We begin with a comprehensive assessment so we can recommend care that truly fits your situation.
Initial Comprehensive Home Assessment
A 90-minute, in-home RN assessment including medical review, home safety evaluation, medication assessment, medical technology support, and a written summary with recommendations.
$425 – one-time visit
*This assessment is required prior to enrolling in ongoing services
Ongoing Wellness Support
Designed for clients who benefit from regular RN monitoring, guidance, and care coordination over time.
Silver – Monthly
One monthly RN visit (50 minutes) focused on vital signs, medication support, and symptom review.
$215/month
(3-month minimum)
Gold – Biweekly
Two RN visits per month with ongoing monitoring, medication oversight, and care coordination.
$425/month
Platinum – Weekly
Weekly RN visits with proactive monitoring, care coordination, and priority support.
$650/month
One-Time & Add-On Services
Available as standalone services or as supplements to ongoing care.
Medical Technology Support Visit (60 minutes) — $225
Family Support Visit (Virtual or In-Person, 60 minutes) — $225
Medication Setup (up to 7 medications) — $145
Additional Medication Support — $20 per medication
Additional Visit Time (15-minute increments) — $55
